Can a Scrotal Ultrasound Diagnose Infertility

While a private scrotal ultrasound in the West Midlands cannot directly diagnose infertility, it does have the ability to highlight crucial causes of fertility issues.

What can a scrotal ultrasound reveal?

A scrotal ultrasound scan can provide detailed images of the epididymis, testicles and other surrounding structures.

This can therefore detect:

  • Spermatoceles or Hydroceles (cysts or fluid collections).
  • Enlarged veins in the scrotum (Varicoceles), which may cause a reduction in sperm quality; however, this is treatable. There are non-surgical and surgical options
  • Obstructions in the tubes that carry sperm.
  • Signs of inflammation, infection, or tumours.
  • Structural abnormalities or smaller testicle/s (Testicular atrophy – shrinking of one or both testicles). Testicular atrophy may be caused by testicular cancer, age, diseases, varicocele, testicular torsion, excessive alcohol usage, orchitis, or hormone imbalances.

While any of the findings detailed above could indicate issues that can cause infertility, further testing is usually required for clinical correlation.

What are testicles?

Testicles or testes are an essential element of the male reproductive structure. Testicles generate sperm and secrete the male hormones called testosterone.

Generally, males have two testicles within the scrotum, and the testicles are usually of the same size. Due to the differences in the vasculature’s anatomy, you can have one testicle extending farther down into the scrotum than the other, and this is referred to as scrotal asymmetry. This is common. According to statistics, about 85% of men have the right testis hanging lower than the left testis.

Various conditions may affect testicles, causing discomfort and potentially health issues or infertility.

What are the symptoms of testicle issues?

TYPICAL SIGNS OF TESTICULAR PROBLEMS:

  • A feeling that the scrotum is swollen or heavy: A scrotal feeling of heaviness or weight could be caused by an abnormal growth, a tumour, hydrocele (fluid buildup), or varicocele ( an enlargement of veins within the scrotum. It can occur in one or both testis and may cause a decrease in sperm production and sperm quality, which in some cases may cause infertility)
  • A persistent dull pain: A mild, ongoing aching in the groin or testicles could be a sign of irritation, pressure from an enlarging tumour, orchitis or epididymitis. This ache never goes away on its own, no matter how minor, having a scrotal or testicular ultrasound scan will not only give you a peace of mind, but will help with early diagnosis.
  • Swelling or lumps: If you feel a lump, or bump, or you have visible swelling, seek medical help as this may be a sign that you have an infection, cysts, or, in more extreme situations, testicular cancer.
  • Sharp Pain: Sudden, acute, or painful sensations in either of the testicles could be caused by infection, injuries, or a condition called testicular torsion. Testicular torsion is where a spermatic cord bends, blocking blood flow to the testis. If ignored, this might lead to irreparable damage.
  • Gynaecomastia, or breast enlargement: Masculine breast tissue growth that is out of the ordinary could indicate hormonal defects, which tend to be correlated with testicular tumours.
  • Chronic Pain in the Lower Back: Although there are numerous reasons for back pain, cancer of the testes that has progressed to lower back lymph nodes may be the culprit in certain situations. It is advisable to have a scrotal or testicular ultrasound scan if back discomfort continues without an established source.
What are the causes of testicular issues?
  • Testicular Tumours (Cancerous or Benign): Testicular tumours might be either cancerous or benign (non-cancerous), and they can cause bruising, lumps, pain, or hormonal modifications that affect the body. Successful treatment depends on early detection.
  • Spermatocele or Epididymal Cysts: Spermatocele or epididymal cysts are fluid-filled sacs that form near the testicle, usually in the epididymis, which is a tube that is twisted behind the testicle. Although they are generally benign, large cysts may be uncomfortable or cause swelling.
  • Infections such as Epididymitis or Orchitis: These could be caused by chlamydia or gonorrhoea, which are sexually transmitted infections. These conditions may cause symptoms like scrotal swelling, discomfort, a high fever, and redness. If left untreated, infections may cause infertility issues.
  • Testicular Torsion: Testicular Torsion is the twisting of a testicle and is a medical emergency that requires immediate treatment. It is characterised by immediate, severe testicular pain, along with swelling and nausea. Testicular Torsion is mostly common between the ages of 10 and 15 (puberty stage). It is crucial to advise young teenagers to report any pain in the scrotum area, even if it feels embarrassing.
  • Atrophy (Shrinkage): This testicular problem is caused by injury, hormonal changes, or trauma. Atrophy can potentially indirectly cause erectile dysfunction due to low testosterone levels, a hormone which is essential for maintaining erectile function.

What is a torsion?

Testicular torsion is when the spermatic cord, which provides the testicle with blood flow, becomes rotated and twisted. This cuts off the blood supply to the testicle and can cause sudden pain and swelling. Emergency surgery is required in order to save the testicle.

Do you offer blood tests relating to the testicles?

What kinds of things can a scrotal testiclar ultrasound detect?

A scrotal testicular ultrasound scan can detect tumours, cysts and infection. Although ultrasound scans can be used for diagnosis, you me need a scan in conjunction with other diagnostic tests.  Your GP may be able to advise on this.

How do I prepare for a Scrotal Testicular scan?

There is no specific preparation for this scan. You can eat and drink as normal and take any medication you are on.

What do I wear?

You will need to wear loose-fitting and comfortable clothing. Your trousers will need to be easily removable as you will need to expose your whole genital area. The sonographer will provide you with a sheet to cover your lower abdomen. The sonographer will make every effort to protect your dignity during the exam.

Do I need to consent to a Scrotal Testicular Scan?

All patients must give their consent (permission) before undergoing an examination. If they are unable to give consent themselves then a person with power of attorney will need to give consent on their behalf. Please ensure that the pre-treatment consent form is completed via the embedded link in your confirmation email before you attend the appointment. The scan will not go ahead if this form is not completed.

Are Scrotal Testicular Scans painful?

Scrotal testicular scans are not painful however it may be uncomfortable if the area is tender as the sonographer will need to gently apply pressure with the ultrasound wand to generate good images.

What are the red and blue blobs on the ultrasound?

During the scan the sonographer may use a colour doppler to check blood flow in your tissue and organs. This will appear as blue and red blobs on the image and are nothing to be concerned about.

What happens during the scan?

The sonographer will put ultrasound gel onto your skin and move a probe over your testicles. They will check the midline of your scrotum and then each testicle. They may check areas multiple times.

How uncomfortable is a testicular ultrasound?

A testicular or scrotal ultrasound is painless; however, you may feel pain or discomfort if you recently had an injury, infection or testicular torsion. To obtain good images, a sonographer may apply slight pressure during scanning and this may be uncomfortable and painful. You will need to let the sonographer know.

Do men get sedation for testicular ultrasound?

Testicular ultrasound does not require sedation as it is not painful and is non-invasive.

Do they shave you for an scrotal ultrasound?
